摘要：目的:系统评价中医定向透药治疗糖尿病周围神经病变(DPN)的疗效。方法:计算机检索中国知网、中国生物医学文献数据库、万方数据库、维普数据库、PubMed、EMbase、the Cochrane Library、Web of Science文献数据库中公开发表的中医定向透药治疗DPN的随机对照试验,检索时限为各数据库建库至2022年5月30日。由2名研究者依据纳入与排除标准独立筛选文献、提取数据,并使用Cochrane偏倚风险工具对纳入的文献进行方法学质量评价后,采用RevMan 5.3软件进行Meta分析。结果:共纳入12项随机对照试验研究。Meta分析结果显示,与常规降糖治疗相比,中医定向透药治疗能提高病人的临床总有效率[OR=4.07,95%CI(2.77,5.98),P<0.00001],提高腓总神经感觉神经传导速度[MD=5.30,95%CI(3.01,7.59),P<0.00001]、腓总神经运动神经传导速度[MD=5.12,95%CI(2.03,8.22),P=0.001],提高正中神经感觉神经传导速度[MD=6.41,95%CI(5.38,7.45),P<0.00001]、正中神经运动神经传导速度[MD=3.61,95%CI(2.30,4.92),P<0.00001],降低中医证候积分[MD=-2.68,95%CI(-3.15,-2.21),P<0.00001)]、多伦多临床系统评分[MD=-2.14,95%CI(-2.37,-1.90),P<0.00001]。结论:现有证据表明,中医定向透药治疗能提高DPN病人的神经传导速度和临床总有效率,降低中医证候积分和多伦多临床系统评分,改善临床症状。受纳入研究数量和质量的限制,此结论尚待更多高质量研究予以验证。
Objective:To systematically evaluate the therapeutic effect of directional penetration of traditional Chinese medicine on diabetes peripheral neuropathy(DPN).Methods:The published randomized controlled trials of Chinese medicine directed penetration therapy for DPN were retrieved from CNKI,CBM,WanFang Database,VIP,PubMed,EMbase,the Cochrane Library and Web of Science.The retrieval time was from inception to May 30,2022.Two investigators independently screened the literature based on inclusion and exclusion criteria,extracted data and evaluated the methodological quality of the included literature using the Cochrane risk of bias tool.RevMan 5.3 software was used for Meta-analysis.Results:A total of 12 RCTs were included.Meta-analysis results show that compared with conventional hypoglycemic therapy,traditional Chinese medicine directional penetration therapy can improve the total clinical effective rate of patients[OR=4.07,95%CI(2.77,5.98),P<0.00001],improve common peroneal nerve SCV[MD=5.30,95%CI(3.01,7.59),P<0.00001],common peroneal nerve MCV[MD=5.12,95%CI(2.03,8.22),P=0.001],improve median nerve SCV[MD=6.41,95%CI(5.38,7.45),P<0.00001],median nerve MCV[MD=3.61,95%CI(2.30,4.92),P<0.00001],reduce TCM syndrome score[MD=-2.68,95%CI(-3.15,-2.21),P<0.00001)]and Toronto Clinical System Score[MD=-2.14,95%CI(-2.37,-1.90),P<0.00001].Conclusion:The existing evidence shows that traditional Chinese medicine directional penetration treatment can improve the nerve conduction velocity and clinical total effective rate of DPN patients,reduce the TCM syndrome score and Toronto clinical system score,and improve the clinical symptoms.Limited by the number and quality of included studies,the above conclusions need to be verified by more high-quality studies.
