摘要:骨生物力学作为生物力学的分支,以工程力学理论为基础,研究骨骼在运动过程中的力学性质、功能和变化规律,明确力学信号对细胞形态、基因表达、应力分布等多个层面的影响,是影响骨稳态的潜在机制,在新骨形成中发挥成血管-成骨耦合作用,促进缺损部位的骨再生。近年研究表明骨生物力可促进内皮细胞(endothelial cells,ECs)、成骨细胞(osteoblast,OB)、骨髓间充质干细胞(bone marrow mesenchymal stem cells,BMSCs)等骨系细胞增殖分化以改善骨稳态,而其具体应用研究仍处于初步且缺乏临床数据支持。因此,本文通过阐释骨生物力学在ECs、OB、BMSCs分化中的作用,综述骨生物力学对骨质疏松、股骨头坏死、骨关节炎及骨折愈合等骨代谢疾病的影响,以期为改善骨稳态提供靶点与方法。此外,中药活性成分如丹参醇、牛膝甾酮、黄芩苷、三七总皂苷等可通过影响骨生物力学的差异性表达来影响成血管、成骨,为临床防治相关骨代谢疾病提供策略。
As a branch of biomechanics,bone biomechanics is based on the theory of engineering mechanics and studies the mechanical properties,functions,and changes of bones during movement.It clarifies the influence of mechanical signals in cell morphology,gene expression,stress distribution,and other levels.It is a potential mechanism that affects bone homeostasis and plays a role in vascular-osteogenic coupling in the formation of new bone,promoting bone regeneration in the defect area.Recent studies have shown that bone biomechanics promotes the proliferation and differentiation of endothelial cells(ECs),osteoblasts(OB),and bone marrow mesenchymal stem cells(BMSCs)to improve bone homeostasis.However,its specific application research is still in the preliminary stage and lacks clinical data support.Therefore,this article explains the role of bone biomechanics in the differentiation of ECs,OB,and BMSCs,reviews the influence of bone biomechanics on bone metabolic diseases such as osteoporosis,femoral head necrosis,osteoarthritis,and fracture healing,and provides targets and method for improving bone homeostasis.In addition,the active components of traditional Chinese medicine,such as tanshinol,ginsenoside,baicalin,and total saponins of sanqi,may influence the differential expression of biomechanics to form blood vessels and bones.This provides strategies for the clinical prevention and treatment of related bone metabolic diseases.
